Chronic diseases such as diabetes, cardiovascular disease, and respiratory conditions are among the leading causes of mortality and healthcare costs worldwide. The increasing burden of multimorbidity necessitates scalable, structured, and personalized care delivery. Traditional systems like DMP, C3-Cloud, and CAREPATH have introduced disease-specific care plans and clinical decision support (CDS), but they remain limited by rigid architectures, backend dependencies, and non-clinician-friendly design. This thesis presents KRONIQ, a configurable and standards-compliant chronic disease management platform that empowers clinicians to define, manage, and execute care flows without requiring software development. KRONIQ introduces a visual editor for disease definitions, care pages, and clinical concepts; a rule-based decision support system implemented in FHIRPath and evaluated directly in the browser; and a modular frontend interface for managing patient episodes and dashboards. The system supports HL7 FHIR standards, SNOMED CT and LOINC terminologies, and CDS Hooks for interoperability. Through integrated internal rules, background data queries, and CDS card actions, KRONIQ enables responsive, guideline-based care execution. A series of user acceptance tests covering diabetes, cardiovascular disease, and multimorbidity scenarios demonstrate its flexibility, usability, and effectiveness. KRONIQ offers a significant advancement toward fully configurable, clinician-authored, and interoperable chronic care solutions.
